New Patient Appointments

Step 1

Obtain a referral from GP or Specialist

Patients must obtain a referral letter from a GP (usually valid for 12 months) or from a Specialist (usually valid for 3 months).

To see a Psychiatrist

To see a Clinical Psychologist

Patients must obtain a Mental Health Care Plan (MHCP) from a GP, or a referral letter from a Psychiatrist.

Step 3

Complete the new patient form below and upload your referral.

Step 4

Upon receipt of your referral, the Clinician who the referral is addressed to or the most suitable Clinician will review your request to ensure they are the best match for your needs. We will then call you to agree on a date and time for your first appointment.

Once your appointment is scheduled an email confirmation will be sent to you with a link to the new patient form. Please ensure that this form is completed prior to your appointment.
